Complete Health With PACE to Hold Open House

In honor of its new partnership, Baptist Health and CareLink are hosting a community drop-in open house on Friday, Oct. 14, from 10 a.m. to 6 p.m. for Complete Health with PACE – a joint venture that provides coordinated care and services for seniors. A ribbon-cutting ceremony will take place at 10 a.m., and tours will be available anytime afterward until 6 p.m.

Complete Health with PACE, a Baptist Health and CareLink partnership, offers quality, comprehensive and coordinated community-based health care and services to the elderly so that those enrolled in the program may continue to live at home instead of transitioning to a nursing home. Some of the care and services include medical, rehab, in-home, adult day care, social work, and transportation.

To be eligible for PACE (Program of All-inclusive Care for the Elderly), a Medicare/Medicaid program, a person must be 55 years old or older; certified by the Department of Human Services to be eligible for nursing home level care; live in the community at the time of enrollment without jeopardizing his or her health or safety; and live within the program’s service area, which includes 20 cities and four counties within the central Arkansas area. If eligible, each client will have a dedicated care team that includes physicians, nurses, therapists, personal care aides, social workers and other professionals. The team works with the enrollee and his or her family to determine what services are best for the patient.
